The Science

Your Body Built a Shell.
We Soften It.

Every breast implant sits inside a thin scar capsule your body forms around it. In 10 to 20% of augmentation patients, that capsule thickens and tightens, a process called capsular contracture. It's graded on the Baker Scale, from soft and natural (Grade I) to hard, painful, and visibly distorted (Grade IV).

Focused shockwave therapy delivers targeted acoustic energy that breaks down the hardened collagen fibers, stimulates new blood vessel formation, and calms the inflammation driving the capsule in the first place. No incision required.

Who This Is For

Built For Grade I and II. Honest About Grade III and IV.

Results depend on how far the capsule has progressed. Tom grades you first, then gives it to you straight.

Best Candidates

  • Grade I or II capsular contracture
  • Noticeable implant firmness or shape change
  • Post-op patients wanting preventative therapy

Considering Alternatives

  • Grade III patients weighing options before surgery
  • High-risk history (prior capsule formation, above-muscle placement)
  • Anyone wanting to delay or avoid revision surgery

Not A Fit

  • Grade IV contracture (hard, painful, misshapen) typically needs surgery
  • Tom will tell you directly if shockwave won't help your case
What To Expect

Six Sessions. Two Weeks. No Downtime.

A short, progressive protocol built around your Baker Grade.

1

Free consultation, Baker Grade assessed

2

Focused ESWT applied, mild tapping or pressure, no anesthesia

3

Intensity increases progressively across sessions

4

Six sessions total, spaced 1–2 days apart

5

Back to normal activity immediately after every session

6

Full series completed in 6–10 days, one trip for out-of-town patients

Why Tom

25+ Years In Tissue.
Honest Before Optimistic.

Tom's clinical background started in cardiothoracic surgery in 1998, work that runs directly through vascular tissue and scar formation, the same biology behind capsular contracture. He uses focused ESWT exclusively, not the shallower radial pulse devices some clinics substitute in.

He grades your Baker Scale carefully and will tell you when shockwave isn't likely to help rather than sell you a series that won't work.

"My protocols are honest before they're optimistic. If shockwave isn't right for your grade, I'll tell you before we start."

What To Expect

How The Capsule Responds

A progressive protocol, not an instant fix. Here's the realistic timeline across six sessions.

Sessions 1–2
Lower intensity, tissue begins responding
Sessions 3–4
Progressive intensity increases, softening becomes noticeable
Sessions 5–6
Peak protocol intensity, most significant change in firmness and shape
Following Weeks
Continued softening as the healing response completes
Ongoing
Preventative sessions available for high-risk patients
The Difference

Focused ESWT Reaches The Capsule. Radial Doesn't.

Radial pulse therapy scatters energy from the surface outward, losing precision by the time it reaches the capsule. Focused ESWT concentrates acoustic energy at the exact tissue depth where the capsule sits. For this application, depth and precision are what determine outcomes. Las Vegas Aesthetics uses focused ESWT exclusively.

Common Questions

Breast Encapsulation FAQ

What is capsular contracture and how common is it?

It's abnormal thickening and tightening of the scar tissue capsule that forms around a breast implant. About 10 to 20% of augmentation patients develop some degree of it, graded on the Baker Scale from soft and natural (Grade I) to hard, painful, and distorted (Grade IV).

Can shockwave therapy treat capsular contracture without surgery?

Yes, for the right candidates. Focused ESWT gets reliable results at Baker Grade I and II. Grade III outcomes vary by severity and duration. Grade IV typically needs surgery.

Is there clinical evidence behind this?

Yes. Published research supports extracorporeal shockwave therapy for reducing pain and softening fibrotic capsule tissue (Heine N, Prantl L, et al., Aesthetic Plast Surg, 2013).

How many sessions do I need?

Six sessions, about 15–20 minutes each, spaced 1–2 days apart, completed within 6–10 days. Intensity starts low and increases progressively.

Does it hurt?

Most patients describe it as mild tapping or pressure. No anesthesia needed, and you're back to normal activity immediately.

What's the difference between focused and radial pulse therapy?

Focused ESWT concentrates energy at precise tissue depth. Radial pulse disperses energy from the surface outward with less precision. Focused is what reaches the capsule.

Is it safe for all implant types?

Yes. Focused shockwave therapy is safe across saline, silicone gel, smooth-shell, and textured implants from all major brands.

When should I start treatment?

Earlier is better. Grade I and II respond most favorably, and early treatment can prevent progression to Grade III or IV.

Can shockwave prevent capsular contracture before it happens?

Yes. Preventative sessions make sense for high-risk patients, including those with prior capsule formation, above-muscle implants, or previous revision surgery.

How does this compare to revision surgery?

Surgery means anesthesia, incisions, recovery time, and standard surgical risk. Shockwave is non-invasive for Grade I and II, with no downtime. Grade IV still requires surgery.

How much does it cost?

Pricing depends on your Baker Grade and case, confirmed at your free consultation. Significantly less than surgical revision. Financing available. Call or text 725-527-8211.
